Student Systems Security Access Request

 
Please complete the form below to request employee access to Banner - Student Module, BDM, GULFLINE, COGNOS, and/or Florida Shines.

All security access requests MUST be submitted by the employee’s direct supervisor.

Please contact the appropriate Banner Data Stewards to request Banner access. Below is a list of authorized requestors for each Banner Module (e.g. Student, Finance, Financial Aid, Accounts Receivable, and General).

To submit a student data report request, all items on the form below must be complete with appropriate signatures and returned to the Office of the Registrar at least two weeks in advance of the needed date.
